If you or someone you love has been diagnosed with HIV we are here to help. Call our office at 905-984-8684 and ask to speak to a Case Manager. You will then set up an intake appointment which can take place at our office, your home or somewhere in the community, it is totally up to you. The intake is about an hour long and our Case Managers can let you know what you will need to bring to the appointment.
I’ve Completed the Intake with a Worker….Now What?
Welcome! So you’ve completed the intake process; what’s next?
You will be assigned a Case Manager who will contact you within a few days of your intake and make arrangements to meet with you. Your Case Manager will open a file for you and make sure that all necessary paperwork is completed. You and your Case Manager will prioritize your needs and goals and put them together into a Support Plan. How often you meet with your Case Manager will be a decision you make together.
